In the Employment Status section, if you choose Employed, Self-Employed or Retired, the Employer fields appear. Otherwise, they are hidden.
I created the dropdowns for Other Investments, Liquid Net Worth and Net Worth. I could not add the actual values in the dropdowns because the pdf you sent me with the highlights did not have working dropdowns. Just send me the values that should be in those dropdowns and I will add them.
The beneficiary and contingent beneficiary sections. Jot forms will allow us to show the fields for one beneficiary with a button to “add additional” if there’s more than one. This will keep the form cleaner and shorter. Since gravity forms doesn’t have this, I just put placeholder fields on this form. Those “configurable lists (https://www.jotform.com/widgets/configurable-list) need to be created after teh conversion is done from gravity forms being used here to jot forms.
